Through routine monitoring and interdisciplinary coordination, CCM ensures timely medication adjustments, lab review, and patient education to support disease control and prevent complications. For hypertension and diabetes, CCM facilitates blood pressure and glucose tracking, reinforcing adherence to pharmacologic and lifestyle interventions. In hypothyroidism, routine thyroid function testing guides levothyroxine titration. Electrolyte imbalances are addressed through frequent monitoring and adjustment of dietary and pharmacologic inputs. Anemia management includes identifying underlying causes, iron or vitamin supplementation, and monitoring hemoglobin trends. Vitamin deficiencies are corrected through tailored supplementation and dietary counseling. Examples of chronic conditions include, but are not limited to, Alzheimer's disease and related dementias, arthritis, asthma, atrial fibrillation, autism spectrum disorders, cancer, cardiovascular disease, chronic obstructive pulmonary disease, depression, substance use disorders, diabetes, hypertension, ... chronic conditions expected to last at least 12 months or until the death of the patient, and that place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline. formulating a comprehensive care plan, interactive remote communication and management (usually over the phone), medication management, and coordination of care between providers.
